KINDERGARTEN REGISTRATION FOR 2010-2011

Registration for kindergarten will take place Monday, April 12 through Friday, April 16, from 8:00 a.m. until 2:30 p.m. each day, by appointment only at the district Enrollment Office located at Maple Heights High School, 5500 Clement Drive. 

Enrollment/Registration packets are available at the main entrance of the high school, Raymond Elementary School, and the Board of Education or can be obtained online by selecting the link below. Packets should be completed prior to your appointment.

The following items are required to complete the enrollment process:

• Birth certificate – Original or Certified Copy (child must be 5 by 9/30/10 to enroll in Kindergarten)**
• Social Security Card or Number
• Custody Papers – A signed and certified copy (if applicable)
• Complete Immunization Record
• Parent/Guardian Ohio Driver's License or Ohio I.D.
• Child's Medicaid Number (if applicable) 
• IEP/ ETR / MFE – If child receives special education services
• Proof of Residency – Such documents include, but are not limited to: voter registration card, municipal income tax return, valid driver’s license with current address, utility bills, credit card statements, pay stubs, current lease, bank statement, and insurance declaration pages. At least three different, recently dated items showing your name at your current Maple Heights address are required.

**Early Entrance Screening. Children whose fifth birthday falls after September 30, 2010, but no later than December 31, 2010, may qualify for early admission in the 2010-11 Kindergarten Program if they show a high degree of maturity in areas of social, cognitive and motor development. One screening session will be held on a date which has yet to be determined. However, if screening criteria are met, additional assessment would be conducted to determine final eligibility for early entrance. Parents may request to have their children participate in the screening by writing to Maple Heights City Schools, Special Pupil Services, 5500 Clement Drive. The letter should include: Child's Name, Birth Date, Parents' Names, Address, Phone Number, and a brief explanation of the reasons for requesting early entrance. Deadline for submitting this information is June 30.
